Housing Connect is seeking applicants to deliver high quality case management for households in Permanent Supportive Housing programs including households living in Project-Based and Tenant-Based locations using State Homeless Funding. Subrecipients will provide specialized housing case management services to chronically homeless, high acuity individuals with disabilities residing throughout Salt Lake County. These case management services will be provided directly to recipients in their own homes, in the community, or wherever they reside prior to finding suitable housing.
Housing Connect has applied for case management funding to subaward and is anticipating funding award at the end of April. Available positions will be modified after the final funding is announced, but please apply for all positions you are interested in providing up to the amounts listed below.
If full funding is awarded, Housing Connect anticipates awarding funding as approximately as follows:
HACSL – New Chronic S+C – SP3 – 3.25 FTE Case Managers to support 49 Households
HACSL – Renewal S+C – SP – 1.5 FTE Case Manager to support 23 households
HACSL – COCR PSH – COCR – 1.25 FTE Case Managers to support 19 households
HACSL – Bud Bailey COC – SPBB – 1.5 FTE Case Managers to support 16 households
HACSL – Grace Mary Manor S+C, HACSL – Grace Mary Manor Section 8, HACSL – Grace Mary Manor Unfunded – GMM – 2 FTE Case managers to support 44 households
