Permanent Supportive Housing (PSH) is a model that combines low-barrier affordable housing, health care, and supportive services to help individuals and families lead more stable lives. Housing Connect has implemented PSH to provide housing to those with the highest barriers to housing.
Once a family is housed in one of Housing Connect’s PSH projects, they receive supportive services—including case management—with the primary goal of helping tenants maintain their housing and maximize their capacity for independent living, while decreasing the likelihood of recurrent episodes of homelessness.
This type of case management focuses on helping tenants meet the obligation of their lease and develop crucial skills to live stably in the community. A key practice of Housing Connect’s service delivery model is that supportive services staff—charged with advocating for tenants and linking them to essential services—work in partnership with property management staff to meet the needs of tenants.
It is our goal that these projects become a part of, give back to, and benefit from the local community by providing safe, affordable housing linked to supportive services that helps people live more stable, productive lives.
